How do I request city to repave street?

I have lived in my neightborhood for 11 years and the street has always been In pretty bad shape and all they do is patch it up but eventually it gets messed up again. This is in the los Angeles county in panorama city to be more specific which Is in the valley of san fernando. There is an elementary school just a block away on the same avenue and ihe street is just as bad but all surrounding streets are nicely paved. can some one please help.
